There are a number of options available to individuals looking for a healthcare plan. If you need long term care services for more than 120 days and prefer to live safely at home, CenterLight Healthcare’s Program of All-Inclusive Care for the Elderly (PACE) can help.
CenterLight PACE offers you an all-inclusive customized plan carried out by a dedicated team that includes a physician, nurse, registered dietitian, social worker, physical and occupational therapist, and therapeutic recreation specialist. Together, these health professionals work with you and your family to help you continue to enjoy living safely in your own home and community.
Eligibility requirements for CenterLight PACE include:
- Being 55 years or older
- Currently Medicaid eligible and/or entitled to Medicare Part A and/or enrolled in Medicare Part B or able to pay privately
- Living in our service area including the Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Western Suffolk and Westchester counties
- In need of nursing home level of care, but able to remain living safely in your home. You must need more than 120 days of community based long-term care services.
CenterLight PACE offers:
MEDICAL CARE
- Primary and specialty medical care, including dental, podiatry and eye care, either at CenterLight’s day centers or in the community
- Nursing services at home or at CenterLight Healthcare PACE’s day centers
- Rehabilitation services, including physical and occupational therapies
- Nutritional counseling
- X-Rays and other laboratory services
- Medications
- Medical equipment (DME)
- Hospital care
- 24/7 emergency medical services
- Nursing home care
HOME CARE
- Aides in the home to assist you with personal care, meals, shopping and other aspects of daily living
- Consumer Directed Personal Assistance Service (CDPAS) available
- Therapeutic home modifications to accommodate your disabilities and special needs
SOCIAL ACTIVITIES
- Recreational activities, including social discussion groups, arts and crafts, parties, trips, cultural and education programs at our day centers
- Beauty/Salon services (available at select sites)
OTHER SERVICES
- Transportation to and from our day centers and medical appointments
- Social work services for participants and caregivers, including counseling focused on health and adjustment issues, help with benefit entitlements and other assistance you may need
- Nutritious meals in our day centers
- Support for family caregivers

Day Health Centers, Close to Your Home
At CenterLight PACE’s day centers, you can participate in enjoyable recreational activities, make new friends and look forward to nutritious meals. Just a few steps from our day centers, participants can also receive a variety of services, including medical care, physical and occupational therapy, social work services, and nutritional counseling, all designed to keep you healthy and supported. We offer translation services and multicultural programming to ensure everyone feels welcome and understood.

RECREATION ACTIVITIES
Participants enjoy more than 100 activities per month at CenterLight PACE sites, including art classes, games, cultural celebrations, and events.
Rehabilitation Therapy
CenterLight Healthcare PACE centers are furnished with the latest rehabilitation equipment. Our dedicated and compassionate staff will work with you to help you gain maximum functioning.
Personalized Care
CenterLight PACE participants can consult with our compassionate physicians and nurses, just a few steps away from the Day Health Center.
Social Work Services
CenterLight PACE social workers help our participants with counseling, entitlements, and other assistance they may need.
Nutrition Counseling
Registered Dietitians are part of the CenterLight PACE care team, and provide nutritional counseling to participants.
Transportation
CenterLight PACE provides transportation to and from medical appointments, as well as to and from our PACE Centers.

Our team of physicians, nurses, social workers and therapists work with you and your family to develop a healthcare plan that’s right for you. We also provide transportation from your home to our day centers, doctors’ appointments, and other health-related needs.

How to Enroll
To get started in the enrollment process for CenterLight PACE, just give us a call at 1-833-252-2737, Monday-Friday, 8:00 a.m. to 8:00 p.m. TTY users should call 711. A member of our team will provide plan education and answer any questions you may have. We can also help if you need assistance with applying for Medicaid.
If you are new to needing Community-Based Long-Term Care Services and Supports (CBLTCSS), a CenterLight Healthcare PACE Intake representative will review your eligibility. Individuals 55+ with active Medicaid and/or have both Medicare and Medicaid and are seeking to enroll in a PACE plan need to choose between the following assessment options, depending on their qualifications:
- Direct Eligibility Assessment by a CenterLight Healthcare PACE Registered Nurse. This option allows PACE plans to conduct an initial assessment directly to determine if an individual meets eligibility requirements. If you meet the qualifications for the PACE Direct Eligibility Assessment Option, our Intake Team
will provide a PACE Direct Eligibility Disclosure Letter. Please review this letter carefully before you make your decision between the 2 assessment options.
- A New York Independent Assessor (NYIA) Assessment by a New York State Registered Nurse. If a NYIA Assessment takes place, the CenterLight Healthcare PACE Registered Nurse will conduct a review of the NYIA Community Health Assessment and together with you and/or your legal representative, develop an Initial Service Plan (ISP).
You have the right to choose the option that best suits your needs.
The participant may be liable for the costs of unauthorized or out-of-PACE program agreement services. Upon enrollment, the PACE program will be the participant’s sole service provider. Participants will have access to all services needed as identified by the Interdisciplinary Team, but not to a specific provider of these services. Please contact us for more information.
