A Day in the Life of CenterLight Participants

For CenterLight Healthcare Program of All-Inclusive Care for the Elderly (PACE) participants,* a good day begins at home, waking up in familiar neighborhoods surrounded by the people, places, and routines that matter most. CenterLight PACE helps participants hold on to those everyday moments while bringing coordinated care, practical support, and meaningful connection into their lives.
Morning: Starting the Day with Confidence
As participants begin their day, they know that a caring team is just a call away. Whether they have a question about medication, an appointment, or how they are feeling, participants can turn to people who know their care plan, respect their preferences, and understand what matters to them.
The morning may start with pre-arranged transportation, which helps participants get to medical appointments or the CenterLight PACE center safely.
At the PACE center, familiar faces welcome them for breakfast. It is more than a place to receive care. It is a community where participants feel known and cared for.

Midday: Care as Unique as Our Participants
Later in the morning, participants may meet with members of their care team. Depending on their needs, they may see a provider, social worker, or dietitian. They may also visit the center’s open gym, where rehabilitation therapists help them regain and maintain movement and function.
Because the team communicates with one another, participants can spend less time repeating information and more time talking about their goals, such as staying steady on their feet and continuing the routines they love at home.
The team looks at the whole picture, including medications, nutrition, mobility, and emotional well-being. When participants’ needs change, their care plans can change with them.
Afternoon: Movement, Friendship, and Joy
After a nutritious lunch, participants may join activities at the center. Therapeutic recreation offerings may include music, arts and crafts, cultural celebrations, games, and field trips. The activities are enjoyable, but they also help support movement, memory, confidence, and a sense of belonging.
Throughout the day, participants have opportunities to build friendships, discover hidden talents, share stories, and enjoy time with peers. For older adults who may have once spent many afternoons alone, these connections can become an important part of feeling engaged and maintaining not only physical, but emotional and mental wellness.
Evening: Heading Home and Looking Forward to the Next Day
At the end of the day, transportation brings participants home. They return with more than a completed appointment list or diagnosis. They leave the PACE center with moments that make them look forward to tomorrow.
