CenterLight Healthcare Program of All-Inclusive Care for the Elderly (PACE) offers eligible adults 55+ residing in the Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Western Suffolk and Westchester counties all the care they need to remain living safely and independently in their own homes. This includes occupational therapy services designed to help older adults regain independence and improve their ability to perform daily activities. Occupational therapy at CenterLight PACE focuses on enhancing our participants’ functional abilities, such as dressing, cooking, bathing, and managing personal care, to help them maintain their independence and a high quality of life. Whether recovering from an illness, injury, or managing the effects of aging, CenterLight PACE’s experienced occupational therapists work closely with each individual, their caregiver, and the rest of the care team to create personalized treatment plans.

Through hands-on exercises and techniques, CenterLight PACE’s occupational therapists aim to improve mobility, strength, and coordination, enabling participants to live safely and independently in their own homes. Occupational therapy also focuses on adaptive techniques and assistive devices that make everyday tasks easier, helping participants remain as self-sufficient as possible.

CenterLight Healthcare has an approved PACE contract with the Centers for Medicare and Medicaid Services (CMS) and NY State Department of Health (NYSDOH). Enrollment in CenterLight Healthcare PACE depends on renewal of its contract with CMS and NYSDOH. Participants may be fully and personally liable for the cost of unauthorized or out-of-PACE program agreement services. Upon enrollment, the PACE program will be the participant’s sole service provider. Participants will have access to all services needed as identified by the Interdisciplinary Team, but not to a specific provider of these services. Please contact us for more information.
