CenterLight Healthcare Program of All-Inclusive Care for the Elderly (PACE) provides eligible adults 55+ residing in the Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Western Suffolk and Westchester counties all the care they need to remain living safely and independently in their own homes. This includes physical therapy services, which can help them regain independence and improve their quality of life. Physical Therapists focus on participants' ability to walk, climb up and down stairs, and their ability to go outside in the community. Whether recovering from surgery, injury, or managing a chronic condition, CenterLight PACE’s skilled rehabilitation therapists create personalized treatment plans that address specific goals and enhance overall physical function.
CenterLight PACE Physical Therapists work closely with participants, their caregivers, and the rest of the care team to help improve strength, flexibility, balance, and coordination using therapeutic exercises, manual techniques, and advanced equipment. A comprehensive assessment of each participant’s physical condition helps develop a personalized plan to promote healing and prevent further injury.
