Rehabilitation Services | CenterLight PACE

CenterLight Healthcare Program of All-Inclusive Care for the Elderly (PACE) provides eligible adults 55+ residing in the Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Western Suffolk and Westchester counties with all the care they need to remain living safely and independently in their own homes and communities. In addition to nursing care, social work, nutrition, therapeutic recreation services, transportation to medical appointments/PACE centers, and other services our participants need, CenterLight PACE offers rehabilitation services. The goal of rehabilitation is to help older adults regain or maintain their independence and improve physical function after illness, injury, or surgery. Our physical and occupational therapy services are personalized to meet the specific needs of each individual. These services are designed to enhance mobility, strength, and coordination, as well as support recovery from conditions such as stroke, arthritis, and orthopedic surgeries.

CenterLight Healthcare PACE’s team of skilled physical and occupational therapists work closely with the rest of the care team and participants to develop customized treatment plans that promote healing, reduce pain, and prevent further complications. Whether in the comfort of their home or at one of CenterLight’s PACE centers, rehabilitation services are delivered with a high level of care and compassion. With a focus on individualized care and ongoing support, CenterLight Healthcare PACE is dedicated to helping older adults live healthier, more independent lives.

CenterLight Healthcare has an approved PACE contract with the Centers for Medicare and Medicaid Services (CMS) and NY State Department of Health (NYSDOH). Enrollment in CenterLight Healthcare PACE depends on renewal of its contract with CMS and NYSDOH. Participants may be fully and personally liable for the cost of unauthorized or out-of-PACE program agreement services. Upon enrollment, the PACE program will be the participant’s sole service provider. Participants will have access to all services needed as identified by the Interdisciplinary Team, but not to a specific provider of these services. Please contact us for more information.
