Keeping Up with the PACE of Aging

CenterLight Healthcare’s Program of All-Inclusive Care for the Elderly (PACE) offers older adults 55+ all the services they need* to remain living independently in their own homes and communities.

CenterLight Healthcare PACE's goal is to enrich the lives of the thousands of New Yorkers we serve by providing essential services for their physical, social, and emotional health.

Our team which includes, but is not limited to, doctors, nurses, physical and occupational therapists, social workers, dietitians, and therapeutic recreation specialists, work together to ensure that our participants receive the care they need, where and when they need it.

PACE Centers, Close to Your Home

Our PACE Centers are located right in the community and offer an array of services that can help older adults keep up with the pace of aging.

Our Services Include:

Primary and specialty medical care, including dental, podiatry and eye care.

Nursing services at CenterLight PACE’s day centers or in the home.

Nutritious meals in our centers and Nutritional Counseling by Registered Dietitians.

Aides in the home to assist with personal care, meals, shopping and other aspects of daily living.

Recreational activities, including social discussion groups, arts and crafts, parties, cultural and education programs at our day centers.

Medications and Medical Equipment (DME).

Rehabilitation services, including physical and occupational therapies, as well as access to our open gym.

Social work services, including counseling focused on health and adjustment issues, help with benefit entitlements and other assistance you may need.

Time Well Spent at our PACE Centers

Eligibility requirements for CenterLight PACE:

• 55 years or older
• Currently Medicaid eligible and/or entitled to Medicare Part A and/or enrolled in Medicare Part B or able to pay privately
• Living in our service area including the Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Western Suffolk and Westchester counties
• In need of nursing home level of care, but able to remain living safely in your home. You must need more than 120 days of community based long-term care services.

The participant may be liable for the costs of unauthorized or out-of-PACE program agreement services. Upon enrollment, the PACE program will be the participant’s sole service provider. Participants will have access to all services needed as identified by the Interdisciplinary Team, but not to a specific provider of these services. Please contact us for more information.
